Nominations Sought for 2008 Family Physician of the Year
LAFP is now accepting nominations for the 2008 Family Physician of the Year Award. Help us bring recognition and visibility to a deserving family physician that serves and benefits the profession, the specialty, and the community.
This award recognizes a family physician who has:
- exhibited extraordinary, beyond-the-call-of-duty merit;
- articulated and encouraged members and students to pursue the ideals of family medicine; and
- conveyed these ideals to the public.
Award recipient will be honored on Saturday, July 5, during the LAFP Awards Dinner, held at Sandestin Golf and Beach Resort in Sandestin, Florida.
Nomination Guidelines:
Nominations may be made by any LAFP member, family medicine residency program or department of family medicine.
Individuals who have been nominated, but not selected in the past can be nominated in subsequent years. Physicians who were nominated but not selected for 2007 awards automatically will be considered as candidates in 2008.
Selection Process:
The LAFP Nominating Committee will select the award recipient based on demonstration of the following criteria:
Family Physician of the Year
- Is an outstanding family physician role model
- Maintains high professional standards and service orientation
- Provides high quality, family-centered continuing healthcare
- Is active in the community, education or other public affairs
- Is an active member in good standing of the LAFP
- Is active in the field of family medicine and conforms to the pledges made by the LAFP
- Is an active participant in local medical society and supports all progress in the medical profession
- Has made outstanding contributions in his/her community in the social, religious, or medical fields
- Has high morals and worthy ideals and practices them
- Is a leader in civic development
- Favors good schools in the community and has an interest in his/her community affairs

Nomination Process:
To nominate an individual, please include the following documentation and support:
- A completed nomination form - you must complete the "Reason for nomination" section of the form explaining why you feel your nominee is special to our state and to the specialty of family medicine
- One letter from a colleague and two letters from patients should be attached to the nomination form. Additional letters of endorsement and support materials are recommended
- Current CV of the nominee and an outline of the physician's activities and involvement in the community and the medical field. Other outstanding contributions to civic or professional organizations should be included
