Call For Family Physician of the Year Nominations!!!

The Nominations Committee urges all LAFP members to consider their colleagues and submit a nomination for the Louisiana Family Physician of the Year (FPOY) Award. This award is one of the LAFP's highest honors. Help us bring recognition and visibility to a deserving family physician that serves and benefits the profession, the specialty and the community.

Critieria

The nominee should:

  • Be an “Active” member, in good standing, of the Louisiana Academy of Family Physicians;
  • Be active in his/her field and conform to the pledges made to the Academy;
  • Be an active participant in his/her parish medical society and support all progress in the medical profession;
  • Have made some outstanding contribution in his/her community in the social, religious or medical fields;
  • Have high morals;
  • Have worthy ideals and practice them;
  • Be a leader in civic development;
  • Favor good schools in the community and have an interest in his/her community affairs.

Nomination Requirements

Supporting Documentation

  • One letter from a colleague and two letters from patients should be attached to the nomination form. (Additional letters of endorsement and support materials are recommended.)
  • All pages must be photocopy-ready and of reproducible quality, and
  • The entire packet should be mailed flat — do not fold.

The Nominating Committee will judge submissions.

Award Presentation
The LAFP Family Physician of the Year Award will be presented to the winner at the Annual Assembly in Lake Charles, LA on July 9 - 12, 2009 during the Installation & Awards Dinner.

Deadline
Nominations and supporting documents are due in the Academy office no later than March 15, 2009.

Special Note
The Louisiana Academy Family Physician of the Year will automatically become the LAFP’s nominee for the AAFP Family Physician of the Year award in the following year.

Call For LAFP Board Nominations

The Louisiana Academy of Family Physicians Nominating Committee is seeking quality leaders who are willing to serve on the LAFP Board of Directors. Board members are expected to attend meetings, and to take part in the decision-making process. They are also required to exercise a reasonable degree of knowledge, skill and care in bringing their best judgment to the performance of their responsibilities. The Board meets four times a year with meetings scheduled during the spring (March/April), summer (Annual Assembly), fall (September/October), and winter (January/February).

It is the duty of the Board members to remain informed on the problems of the Louisiana Academy; to study the materials that are distributed to them; and to exercise sound judgment in arriving at decisions. The Board of Directors exercise the ultimate executive responsibility and authority of the Louisiana Academy. The duty of the Board is to act in the best interest of the Academy. The deadline to submit a Board nomination is March 15, 2009.
