American Academy of Family Physicians (AAFP) recently released a patient-centered medical home presentation, “The Patient-centered Medical Home (PCMH): the Family Medicine Model.”
AAFP President Dr. Ted Epperly delivered the presentation to Academy leaders and staff at the AAFP Annual Leadership Forum on April 25 in Kansas City, Mo. LAFP leaders and staff were in attendance.
This presentation gives a simplified definition and explanation of the PCMH. There also is a PCMH handout to coincide with the presentation. All Academy members are encouraged to reference and use the presentation and handout for any presentations or PCMH education in their community.
