Tool to Educate Patients about the Patient-centered Medical Home
TransforMED, in conjunction with Emmi Solutions and Patient Centered Primary Care Collaborative (PCPCC), has launched a nationwide initiative aimed at educating patients about what it means to be part of a patient-centered medical home (PCMH).
As part of this initiative, TransforMED created an easy to use, online educational tool to support this endeavor. This Web-based tool will help patients understand the value of the PCMH and seek to engage patients in their healthcare partnership with their primary care physician.
