The LAFP Foundation Needs YOUR Help!!!
The LAFP Foundation fundraisers are in full swing, but your help through donations and sponsorship is vital! The Foundation's 16th Annual Silent Auction, Foundation Golf Tournament, and Foundation Tennis Tournament will be held during our 61st Annual Assembly in Destin, Florida, and are quickly approaching! However, without your involvement and support, it will not be a success. Proceeds will benefit student and resident members. For information and how you can participate in each of these important events, see below:
Donate an Item to Benefit the 16th Annual Silent Auction!!
The Louisiana Academy of Family Physicians Foundation is asking for your assistance in supporting their 16th Annual Silent Auction.
Please complete the Donation Form and return to the LAFP Foundation, 919 Tara Blvd., Baton Rouge, Louisiana 70806, by June 1, 2008 , so that we may complete our records, provide receipts and prepare the Bid Sheets. We ask that you use one sheet per donation . Please join us in making this event a successful event!

Support the Foundation’s fundraising efforts by coming out for a Luau, where we will have wonderful items to bid on. There will be music and a buffet type meal for this event. Bring the kids and come support your Foundation. Ticket prices are $50 adult, $25 child. Adult and Child Guest registrations include one ticket. To purchase additional tickets, please complete the ticket order form.
Foundation Golf Tournament, Thursday, July 3, 2008
1:30 pm - 5:30 pm - Sandestin Golf and Beach Resort - Links Golf Course
Don't miss this great chance to "tee off" with fellow colleagues. The cost of the event is $125 per person and includes 18 holes of play, golf cart, refreshments and prizes. Please consider supporting the Foundation and playing in this fun and relaxing tournament. To register, fill out a Registration Form and fax to 225-923-2909.

Not interested in playing? Consider providing a Hole Sponsorship of $150.

Ready, set, MATCH! Plan on some court action with colleagues! The cost of the event is $50 per person and includes court rental, refreshments and prizes.
