Because you are often the first to notice signs of cognitive decline, the Cognitive Care in Family Medicine Settings Quality Improvement project is designed to help family medicine teams integrate cognitive evaluation, diagnosis and management into everyday clinical workflows.
Participants receive education, training and practical implementation strategies to support earlier identification of mild cognitive impairment and dementia in primary care. Using an Office Champions model, practices designate a team member to help lead the implementation of evidence-based strategies that support cognitive care at the point of care. Findings will be shared on how the practices use the tools and what is most effective.
Through this project, participating practices can strengthen their approach to cognitive evaluation while helping the AAFP and the University of Washington identify barriers and gaps that affect early diagnosis in family medicine settings.
